Appointments
To make an appointment, please call reception at your home Qliance clinic. We will make every attempt to schedule an appointment with your personal Qliance clinician, however if your provider is unavailable, we will schedule you with another clinician.
Weekend Appointments
We try to reserve as many weekend appointments as possible for urgent care needs, but understand that some patients can only be seen on weekends. In such cases, we will do our best to accomodate your scheduling needs.

Prescription Refills
To refill a prescription, please call (206) 913-4720 or send an email to refills@Qliance.com. For accurate and efficient processing of your refill, please be sure to include the following information:
- Name of Qliance clinician
- Name of medication
- Dosage (e.g. oz, mg)
- Frequency (e.g. daily)
- Desired quantity (e.g. 30 or 90 day supply)
- Preferred pharmacy name & phone number
Please note: If your Qliance clinician is unavailable on the day of your request, processing your refill may take up to 48 hours. Refills for controlled substances must be completed by your own clinician and may take up to 3 business days to process.
Patient Forms
To update your Qliance account, request a copy of your medical record or cancel your membership, simply complete the corresponding form below and submit it to Qliance. Mail, fax or email it to info@Qliance.com. Upon receipt, Qliance Member Services will process your request and send you confirmation upon completion.

Patient Care Managers
Cristi Russo
Clinic Operations Manager
Ms. Russo brings extensive strategic health care experience working in hospitals, physician groups and health care organizations to Qliance. As Business Project Manager at the Seattle Cancer Care Alliance, Ms. Russo helped implement two specialty clinics; the Breast Health Clinic and the Pancreas Cancer Specialty Clinic. As an Administrative Fellow at Providence Health & Services, she was responsible for managing multiple executive projects, including operations and satisfaction metrics, program development and providing strategic research and analysis. Ms. Russo worked as a Project Specialist at Children’s Hospital & Regional Medical Center where she helped coordinate, design and launch a large capital campaign among other fundraising initiatives. Previously, Ms. Russo supported the Office of the President of Providence Health and Services in a Project Specialist Role. Here she managed large-scale strategic and analytical projects in support of system initiatives.
Ms. Russo holds her BA in Communications from the State University of New York, College at Geneseo. She received her Masters of Health Administration (MHA) at the University of Washington and completed her capstone in Leadership Development at Group Health Cooperative.

Pat Huber
Member Services Manager
Pat Huber brings extensive experience in customer service, human resources and operations to Qliance. With over 20 year’s experience, Mrs. Huber has a reputation of organization, helping companies improve procedures, implementing policy and increasing profitability. After joining Grange Insurance in June 2000 as a Support Supervisor, Mrs. Huber was promoted to Operations Manager one month later. While at Grange, Mrs. Huber excelled in recruiting, training and retaining excellent employees. Mrs. Huber brings a passion for people, energy and a positive attitude to Qliance and is excited to help Qliance grow nationwide. She received her BA in English and Education from Seattle Pacific University.
