Appointments
To make an appointment, please call the Front Desk at your home Qliance clinic. We will make every attempt to schedule an appointment with your personal clinician, however if your provider is unavailable, we will schedule you with another clinician.
Weekend Appointments
We try to reserve as many weekend appointments as possible for urgent care needs, but understand that your schedule may allow for visits only during the weekend. When this happens, we will do our best to accomodate your appointment request.

Prescription Refills
To refill a prescription, please call (206) 913-4720 or send an email to . For accurate and efficient processing, please include the following information:
- Name of Qliance clinician
- Name of medication
- Dosage (e.g. oz, mg)
- Frequency (e.g. daily)
- Desired quantity (e.g. 30 or 90 day supply)
- Preferred pharmacy name & phone number

Please note: If your clinician is unavailable on the day of your request, processing your refill may take up to 48 hours. Refills for controlled substances are completed only by your personal clinician and may take up to 3 business days to process.
Patient Forms
To update your Qliance account, request a copy of your medical record or cancel your membership, please complete the corresponding form below and submit it to Qliance. Mail, fax or email it to . Upon receipt, Qliance Member Services will process your request and send you confirmation upon completion.
