Is Qliance right for you?

When you become a QlianceOne patient, you are entitled to the services described below for conditions appropriate to primary care. Items written in italics (») indicate incidental services or supplies for which there may be an additional (near cost) charge. In all cases, incidental fees, and their prices will be fully discussed with you in advance of being incurred.

Urgent Care

Qliance strives to provide same or next-day evaluation for urgent medical problems including care of minor injuries and diagnosis and treatment of those problems appropriate to primary care (i.e. conditions that don’t require emergency room, hospital or specialist care).

Preventive Care

Qliance provides thorough periodic physical exams and health screenings for adult men and women. These include evaluation of current state-of-health and recommendations to improve your future health and wellbeing. Qliance provides well-child exams for infants and children as well as sports and other activity participation physicals for individuals of all ages. In Washington state, all state-approved childhood immunizations are provided at no extra cost to the patient.

» Adult vaccines and child vaccines not a part of the Washington state approved program are provided at an additional cost (the injection is an included service, but you may be charged an additional fee (near cost) fee for the vaccine itself).

Chronic Disease Management

Qliance incorporates national guidelines and scientific analysis to provide our patients with state-of-the-art primary care for hypertension, diabetes, hyperlipidemia, heart disease, asthma, arthritis, osteoporosis and many other chronic conditions including primary care of some mental and behavioral health disorders. Your primary care team is available to provide education and training to manage your chronic condition. If your chronic condition changes and you need more specialized care than your care team can provide, we will refer you to an appropriate specialist healthcare provider (see Coordination and Referral section below).

Coordination and Referral

When more specialized care is needed, we are available to assist in referrals to other healthcare providers (including hospitals, specialists’ offices and ancillary health care service providers). With your authorization, we will send each specialist a summary of your medical background and current issues. When you have completed your specialist evaluation, we will provide appropriate medical follow-up. Our goal is to collaborate with your specialists to promote continuity of your care. Your Qliance provider will be available to coordinate all of your care across the healthcare system.

» Emergency room, hospital and specialist care are not included in your monthly care fee. You will be responsible for the cost of care and services provided by non-Qliance medical providers.

Diagnostic Imaging

Qliance providers are able to perform several common x-rays in our offices (including but not limited to, abdomen, sinuses, extremities, neck and back). There is no additional charge for these in-office x-rays.

» In some situations, your Qliance provider may determine that it is clinically appropriate to have your x-ray image interpreted by an outside radiology specialist. If this occurs, you will be responsible for a modest radiology interpretation fee.

Laboratory Testing

Qliance providers are able to perform selected laboratory tests in our offices (including, but not limited to, urinalysis, complete blood count, pregnancy testing, strep testing, glucose testing, prothrombin time/INR, basic chemistry tests and cholesterol). While some of these tests are provided at no additional charge (including urine pregnancy test, urinalysis with or without microscopy, vaginal wet mount (microscopy), finger stick glucose measurement, rapid strep A test, rapid mono test, and KOH prep to examine for yeast), some tests will be charged to you at or near our cost.

» In some situations, your Qliance provider may determine that it is clinically appropriate to order lab tests that are not performed in our office and are instead sent to an outside clinical laboratory. If this occurs, you will be responsible for the cost of their services.

Office Procedures

Qliance providers are able to perform selected medical procedures in our offices (including, but not limited to cardiac exercise treadmill testing, skin biopsies, joint injections, electrocardiograms, colposcopy, IUD insertions, and toenail removals). There is no additional charge for performing these in-office procedures.

» In some situations, your Qliance provider may determine it is clinically appropriate to send specimens collected from procedures to a pathology specialist for evaluation. If this occurs, you will be responsible for the cost of their services. In other situations, your procedure may require items such as IUDs and other durable medical supplies for which you will be charged a (near cost) fee.

Office Medications

Qliance providers are able to administer selected medications in our offices (including but not limited to, most injection medications, ibuprofen, acetaminophen, aspirin, and IV hydration solutions). There is no additional charge for these in-office medications.

» A few medications and some immunizations (as noted above) may require an additional (near cost) fee.

Pharmaceuticals

» Prescription medications are not included in your monthly fee. Prescriptions written by our physicians, nurse practitioners or physician’s assistants can be filled at the pharmacy of your choice.

The following types of services are not provided by Qliance except for basic workup, primary care level treatment and counseling.
