QlianceOneTM Fees

To become a QlianceOne patient, all you have to do is register, pay a one-time registration fee and your first months care fee. Thereafter, your monthly care fee will be processed as per your preferred payment method.

Incidental Fees

Your monthly care fee covers all of our services. However, at times your care may require outside goods or services that are not covered. You may be asked to pay additional fees for goods such as prescription medications, laboratory reagents, or supplies, and services such as outside laboratory tests and x-ray interpretation by an outside specialist if needed. Payment Options

Monthly care fees may be paid by credit or debit card, monthly invoice or automatic funds transfer from your checking or savings account. Monthly care fees are due on the 1st of each month that care is received. For example, a payment made on January 1st pays for care for January 1st to January 31st .

Service Contract

There is no service commitment (or contract) when you join Qliance. Therefore, you may stop receiving care from our practice at any time. However, in order to be fair to our patients who maintain a continuous relationship with Qliance, if you quit and then decide to re-join at a later date, you may be required to pay the registration fee to re-activate your patient medical record. Also, since we limit the number of patients at our practice, we cannot guarantee that there will be room in the practice (or with your provider) when you re-register. Terms for re-registering at Qliance will be discussed with you at the time you cancel your service.

Notes*

The registration fee is per billing account, not per person on the account. Child discount applies to children (ages 0-19) of adult Qliance patients. Fee schedule effective September 1, 2007, per patient, and subject to change.
