__________________________________________________________________________________

Preventive Care

Qliance provides thorough annual exams and health screenings for men and women, infants through seniors. Your clinician will evaluate and talk with you about your current state-of-health and recommend improvements for your future wellbeing.

Wellness Care

In addition to primary care, your Qliance clinician also offers lifestyle and wellness care. Your visits may include extended consultations and personalized coaching to create positive lifestyle changes such as weight loss, smoking cessation and stress management.

Chronic Disease Management

Hypertension, diabetes, hyperlipidemia, heart disease, asthma, arthritis, osteoporosis and many other chronic conditions can be managed effectively at Qliance. Your Qliance clinician may provide education and training to manage your condition, but should you require a specialist's care, we will refer you to a qualified physician who can better manage your care. Your Qliance clinician will continue to monitor and coordinate your specialty care while helping you make health care decisions.

Specialist Care Coordination

Should you need to see an outside specialist, your Qliance clinician will coordinate your referral, correspond with your specialist about your care and will work with you to manage your after-care. If you have insurance, we will strive to provide referrals to providers covered by your policy. If you are paying out-of-pocket for your medical care, we will try to provide referrals to health care providers who have a policy of providing discounts to cash paying patients.

Urgent Care

Qliance provides same or next-day care for urgent medical problems including x-rays, casts, stitches, diagnosis and treatment.

Hospital Care Coordination

Qliance Level 1 Patients

Qliance Level 1 patients will receive remote coordination and supervision of their care via a hospitalist. A hospitalist is an on-site liaison between your primary care provider and hospital physicians. Your Qliance clinician will work with the hospitalist  to coordinate your treatment and provide appropriate follow-up care. If you have insurance, your hospital care may be covered by your policy. Please check with your insurance carrier for coverage details. Please note that your Qliance membership does not include the cost of the hospitalist or any other hospital care.

Qliance Level 2 Patients

Qliance Level 2 patients will receive on-site hospital coordination and personal supervision of their care. Your Level 2 clinician will monitor your hospital admission and specialist coordination while monitoring your charts, procedures, laboratory, imaging and pathology results. Your level 2 clinician will assist in planning your discharge and your follow-up care. If you have insurance, your hospital care may be covered by your policy. Please check with your insurance carrier for coverage details.  Please note that your Qliance membership does not include the cost of the hospitalist or any other hospital care.

After Hours Coverage

Qliance Level 1 Patients

Qliance Level 1 clinicians share after-hours phone coverage. Each Qliance Level 1 clinician has secure, remote access to your medical record to ensure that you after-hours medical needs can be efficiently and effectively met.

Qliance Level 2 Patients

Qliance Level 2 clinicians share after-hours phone coverage. Each Qliance Level 2 clinician has secure, remote access to your medical record to ensure that you after-hours medical needs can be efficiently and effectively met.

X-Ray Imaging

Qliance performs basic x-rays in our clinics and, to guarantee the highest level of quality and safety, sends them to an outside radiologist for professional interpretation. The radiologist fee is $17, payable at the time of service. For patients with Medicare or Medicaid, the radiologist may bill your insurance directly.

Laboratory Testing

Qliance performs blood draws and sample collection in our clinics which are sent out and processed at our partner laboratory. Blood draws and sample collection is free of charge, however tests are offered at cost or may be billed by our lab partner to your insurance.

Office Procedures

Qliance offers selected diagnostic and urgent care procedures in our office including but not limited to: blood draws, EKGs, casts, stitches, joint injections, skin biopsies, skin tag and wart removal, wound care, PAP smears, IUD insertion and spirometry. There is no additional fee for performing these procedures. If a procedure requires durable medical equipment or materials (e.g. crutches or a sling) it is charged at cost. As with all out-of-pocket expenses, their relevance to your care and their cost will be discussed with you beforehand.

Prescription Drugs and Medications

Qliance carries a small supply of generic medications and may dispense them at your clinician's discretion. Prescription drugs dispensed on-site are charged at cost and are generally offered for “first fills” only. Please note that your monthly care fee does not cover the cost of these or any other drugs, however prescriptions filled at your pharmacy may bill your insurance directly.

Vaccines

Qliance clinicians are able to administer selected vaccines in our clinics including seasonal and H1N1 flu. While some vaccines are provided at no additional charge, some are out-of-pocket and are charged at our cost. In Washington State, all state-approved childhood (ages 0-19) immunizations, except HPV, are currently provided at no extra cost.
